AFSA members can activate their union benefits and receive their official membership card by completing the Combined Benefits Form online. This quick process ensures your benefits are assigned and your beneficiary information is on file.
Follow these 3 steps:
Go to https://afsabenefits.org/ and click the blue REGISTER button. Enter your email address and create a password.
Check your email for an activation message. Click the link in the email, then log in using your email address and password.
Click on the Combined Benefits Form, complete it, and submit.
After submitting the form, you will receive:
Your digital AFSA membership card
A PDF summary of your AFSA member benefits
Completing this form is the only way to receive your official AFSA membership card and ensure your benefits and beneficiary details are properly recorded.
